What to Expect at Your First Counseling Session

The short answer

Your first counseling session is mostly a conversation. The therapist wants to understand what brought you in, what you would like to change, and a little about your background, so the two of you can shape a plan together. There is no test to pass, and you never have to share more than you are ready to.

Before the session

You will usually fill out a few intake forms and share your insurance or Medicaid details so coverage can be confirmed. If your session is by video, pick a private, quiet space and test your connection a few minutes early. That is really all the preparation you need.

What the therapist will ask

Expect open questions rather than an interrogation. Your therapist may ask what is going on right now, how long it has been happening, what you have already tried, and what a good outcome would look like for you. They are listening for the shape of your concern, not judging you.

What you can ask them

This is your appointment too. It is completely fair to ask about their experience with your concern, how they usually work, how often you might meet, and what confidentiality does and does not cover. A good therapist welcomes these questions.

What the first session is not

It is not a lecture, and it is not an instant fix. You will not be handed a diagnosis and a solution in one visit. It is also not a place to be judged. The first session is simply where you and the therapist decide whether the fit feels right and sketch a direction.

How to get the most out of it

Jot down a few notes beforehand about what has been bothering you and what you want help with, so you do not forget in the moment. Be as honest as you can, since the therapist can only work with what you share. And notice how you feel talking to them, because fit matters as much as credentials.

What happens after

By the end, you will usually have a sense of next steps, whether that is a regular meeting time, a focus for the coming sessions, or a referral if a different kind of support would serve you better.

Frequently asked questions

How long is the first session?

Usually about 45 to 60 minutes, though the first visit can run a little longer with intake.

Do I have to talk about everything right away?

No. You share at your own pace, and it is fine to leave some things for later sessions.

What if I do not click with the therapist?

That is okay and more common than people think. Fit matters, and it is reasonable to try someone else.

Should I prepare anything?

A few notes on what is bothering you and your insurance details are plenty.

Is what I say confidential?

Yes, with limited legal exceptions for safety. Your therapist will explain those up front.
