The short answer
A DOT SAP evaluation is a face-to-face assessment by a Department of Transportation qualified Substance Abuse Professional that a safety-sensitive worker must complete after a failed or refused drug or alcohol test. The SAP decides what education or treatment you need before you can return to a safety-sensitive job.
It applies to CDL drivers, pilots, transit and rail workers, pipeline crews, and other DOT safety-sensitive employees who tested positive, refused a test, or otherwise violated 49 CFR Part 40. Only a qualified SAP can begin the return-to-duty process. A regular therapist or family doctor cannot sign off on it.
The SAP interviews you about your substance use history, the violation, and any related concerns, then recommends education, treatment, or both. The SAP stays neutral. Their role is to protect public safety, not to advocate for the employer or for you.
You complete the recommended program, return for a follow-up evaluation, and if the SAP agrees you are ready, they release you for a return-to-duty test and set a follow-up testing schedule that can run one to five years.

No. The SAP does not provide your treatment. They assess you, refer you to the right education or treatment, and confirm you completed it. Keeping those roles separate is a federal requirement.
The first evaluation is usually a single appointment. Total time depends on the program the SAP recommends, which can be a short education course or a longer treatment plan.
Yes. A Plus offers DOT SAP evaluations by secure video for clients across Nevada, which is often faster than waiting for an in-person slot.
